The CPC+* and PCMH** value-based care programs were designed to test improved provider payment and service delivery models. The goals are better care for patients, smarter spending and healthier communities.
Payment redesign offers greater cash flow and flexibility for primary care practices to deliver high-quality, whole-person, patient-centered care. It also lowers the use of unnecessary services that drive total care costs.
2018 CPC+ and PCMH year-end reports are being posted this week in the provider My Account portal. We will soon be awarding incentive payments to practices who met performance and quality goals for the year.
