At QualChoice, our goal is to make health insurance simple and to make healthcare better. We’re working with providers, payers and state and federal groups to move from volume-based to value-based care.
With value-based care, provider payments are partially based on the quality and efficiency of the care. Providers are rewarded for working together, for providing the right care and for achieving positive outcomes.
The Patient-Centered Medical Homes (PCMH) program was developed to:
- Improve patient care
- Improve population health
- Lower healthcare costs
Participating Primary Care Physicians are rewarded with:
- Population-based care management fees
- Performance-based financial incentives
These fees and incentives are outside the regular fee-for-service payment model.
QualChoice also participates in the Comprehensive Primary Care Plus (CPC+) program. We were one of the first payers to award performance payments for the 2017 CPC+ program. We look forward to doing the same for the PCMH program.
